Uniform Commercial Code (UCC) paper forms use the International Association of Commercial Administrators (IACA) standard forms and are required to be mailed in with correct payment to the department for filing. Please be sure to check the fees pages to determine payment needed. Most filings are also available through the online portal and cost half the fee or less compared to filing by paper.
