Application:
- Form #296 - Charitable Organization Registration Application
- Use this form to apply for a charitable organization credential number if your organization wants to solicit contributions in Wisconsin or have contributions solicited in Wisconsin on its behalf.
**March 2024 update: Recent legislation modifies certain requirements for financial reporting on fiscal years that begin on or after March 23, 2024. Therefore, two versions of each financial reporting form appear below: one for reporting on fiscal years that began on or after March 23, 2024, and one for reporting on fiscal years that began before that date. Use the proper form for the fiscal year that is being reported on.Financial Reports (Annual): Use these reports for fiscal years that began before 3/23/2024
- Form #1943 - Affidavit in Lieu of Annual Financial Report
- A charitable
organization must submit this form if it received $25,000 or less in
contributions during its most recently completed fiscal year. OR
- A charitable
organization received less than $50,000 in contributions during its most
recently completed fiscal year and only solicited contributions in the
county where its principal office is located.
- Form #1952 - Wisconsin Supplement to Financial Report
- A charitable organization should submit this form if it files an IRS 990, 990-EZ, or 990PF and doesn't qualify for Form #1943.
- Form #308 - Charitable Organization Annual Report
- A charitable organization must submit this form if it does not qualify for Form #1943 or Form #1952
- Form #1953 - Waiver for Audited/Reviewed Financial Report
- A charitable organization may apply for waiver from the requirement of an audited or reviewed financial statement if they meet the requirements provided within this form

Fundraising Counsel (FRC) and Professional Fundraiser (PFR) Forms

- Form #1941 - Professional Fundraiser Solicitation Notice
- A Professional Fundraiser must file this form and a contract for each charitable organization they perform services for.
- As well each charitable
organization shall file written confirmation of the Solicitation Notice
and contract are true and complete to the best of its knowledge.
Professional Employer Organization(PEO) and Professional Employer Group (PEG) Forms
Applications:
- Form #2840 - Application for Registration
- A PEO/PEG should use this form to if it wants to operate in Wisconsin.
- If a PEO/PEG will have a limited presence in Wisconsin, it may qualify to use form #2858 instead.
- If a PEO/PEG has been approved by Employer Services Assurance Corporation (ESAC), it should use form #2854.
- Form #2858 - Application for Registration - Small Operations
- A PEO/PEG should use this
form if it wants to operate in Wisconsin and qualifies for small
operations registration. (Note: a registrant that holds a small
operations credential number does not need to maintain a surety bond or
submit audited financial statements to the department on an annual
basis).
- PEO/PEG must be registered as a PEO/PEG in another state
- PEO/PEG cannot maintain an office in Wisconsin
- PEO/PEG cannot solicit clients that are in Wisconsin
- PEO/PEG cannot have more than 50 employees performing services for Wisconsin clients on any given day
- Form #2854 - Assurance Organization Certification
- A PEO/PEG must use this
form to apply if it has been approved by Employer Services Assurance
Corporation (ESAC) and wants to operate as a PEO/PEG in Wisconsin.

- Form #2843 - Surety Bond
- All Professional employer organizations and groups that submit application form #2840 must also submit a surety bond.
